Smith was last seen outside the Veterans Administration Hospital at 820 S. Damen, clean shaven, wearing a light blue knit cap, black jacket, black pants and black shoes with white soles.
Smith suffers from diabetes and possible dementia and is currently on various medications. He moved into Bronzesville Nursing Home at 3400 S. Indiana Ave. about one month ago. Prior to that, Smith lived at 6230 S. Dorchester and still frequents that area.
