Rey Osorio disappeared yesterday from Cedar Pointe Nursing Home on W. Cermak Road.
He was last seen wearing a dark blue T-shirt and gray jogging pants.
Osorio suffers from several conditions including schizophrenia, bipolar disorder and seizures when he is not regularly taking his medications.
Anyone who may see Osorio should be cautious, avoid approaching him and call Cicero police.
