Homewood firefighter Brian Carey, 28, was killed while battling a house fire in March. Wendell Elias, 87, also died. A second firefighter suffered severe burns.
The Southtown Star reports that an investigation by the National Institute for Occupational Safety and Health says the Homewood Fire Department was not properly prepared to handle the fire.
The report cites several errors including ineffective fire control tactics, insufficient staffing and failure to recognize and react to deteriorating conditions.
Carey was the first firefighter to be killed in the line of duty in the Homewood Fire Department's 109-year history.
