Indiana officials will not fine 'Transformers 3' production crews or Paramount Pictures over a car crash that seriously injured Chicago extra Gabriela Cedillo.Cedillo was driving her own car when she was struck on the head by a piece of flying metal during a stunt sequence for Transformers 3. She has brain damage from the accident.

An investigation by the Indiana Occupational Safety and Health Administration determined the September 1 accident occurred because welding connecting a car to a tow cable for a stunt gave way. The weld was made by a certified welder, according to Indiana OSHA, and all the necessary safety precautions were in place.

Indiana OSHA spokesman Mark Lotter said the crash was "an unfortunate and unforseeable accident."
