Thirty-two-year-old Glen Ray Higginbotham, of Braidwood, was arrested in Joliet Saturday.
Higginbotham's driving privileges were revoked in 2000 after he was found guilty of causing the death of Candace Graham.
Police say Higginbotham was intoxicated from both alcohol and cocaine when he slammed into a car carrying Graham.
His latest arrest comes less than two months after he pleaded guilty to driving with a revoked license.
