The report, released by the Illinois Department of Children and Family Services, contends that Hartgrove Hospital was often understaffed and over capacity. And this, the agency said, created opportunities for attacks by patients on other patients and hospital staff.
The state stopped placing wards at the facility this summer.
Hartgrove's parent company said, "the facility has many more success stories to its credit than the negative ones highlighted in the report."
