ABC 7 and Walgreens Lights Festival Sweepstakes

1. HOW IT WORKS: No Purchase Necessary. To enter, visit the Walgreens booth at the Magnificent Mile Lights Festival on Saturday, November 17th, 2012 between 11am and 4pm and fill out the Walgreens Official Entry Form in full to include the following information: Your First Name, Last Name, gender, address to include city, state and postal code, day time phone number, and birth date. All entries must be postmarked and received by 4pm on Saturday, November 17th, 2012. Limit one entry per person, mailing address or e-mail address (regardless of more than one person uses the same mailing or e-mail address.) Sponsor is not responsible for lost, late or misdirected mail or e-mail. All entries remain the property of sponsors and will not be acknowledged or returned. Four winners will be selected on or about Monday, November 19, 2012.
2. ELIGIBILITY: One (1) entry per person. Any individuals (including but not limited to employees, consultants, independent contractors, and interns), who have, within the past six months, performed services for WLS Television, Inc, Walgreens, any organizations responsible for sponsoring, fulfilling, administering, advertising or promoting the contest or supplying the prize, and/or their respective parent, subsidiary, affiliated and successor companies, and immediate family and household members of such individuals, are not eligible to enter or play. "Immediate family members" shall mean parents, stepparents, children, step-children, siblings, step-siblings, or spouses. "Household members" shall mean people who share the same residence at least three months a year. Void where prohibited. Entrants must be residents in the viewing areas of Illinois, Indiana, Michigan or Wisconsin.
3. TO CLAIM PRIZE: Four First Place Winners and Ten Second Place Winners will be selected on or around Monday, November 19, 2012. The winners will be contacted by telephone and/or U.S. mail by WLS-TV. Prize Release Forms will be sent overnight. Prize fulfillment will be coordinated between the winner and WLS-TV/ABC 7 Chicago once forms are completed and returned to WLS-TV/ABC 7 Chicago. No substitutions or transfer of prizes will be permitted. If winner is under 18 years old, prize will be awarded to parent or guardian. Unclaimed prizes will not be awarded. To claim prizes, (a) winner must provide proof of age, identity and residency, (b) winner must sign and return Affidavit of Compliance; Advertising Release and Prize Release Forms no later than Friday, November 30, 2012.
4. VERIFICATION: Winning entry is subject to verification. Any entry which is ineligible, mutilated, altered, duplicated, tampered with or was not obtained legitimately is null and void. All entries become property of WLS-TV/ABC 7 Chicago. WLS-TV/ABC 7 Chicago is not responsible for failed, partial or garbled transmissions, or for failures of any kind. If for any reason the contest is not capable of running as planned, including tampering, unauthorized intervention, fraud, technical failures, other causes beyond sponsor's control that corrupt or affect the administration, security, fairness, integrity or proper conduct of the sweepstakes, WLS Television, Inc. reserves the right, in its sole discretion to disqualify any individuals who tampers with the entry process, and to cancel, terminate, modify or suspend the sweepstakes.
5. AVAILABLE PRIZE/OFFER: Odds of winning are based on the number of entries received between 11am and 4pm on Saturday, November 17th, 2012 at the Walgreens booth at The Magnificent Mile Lights Festival. Unclaimed prizes will not be awarded.
Four (4) First Prize Winners will be awarded a "Walgreens Photo Fun Gift Basket" to include a Walgreens 2GB Mobile microSD Card with adapter, 2 photo frames, Walgreens photo paper, a Walgreens 35mm single-use camera with flash, an iPhone 4S protective case, one set of 18 image pack Holiday Greeting Cards, Walgreens vitamin supplements, Walgreens thermous, a Walgreens Mobile Quickprints coupon and a $25 Walgreens gift card.
Ten (10) Second Place Winners will be awarded a $50 Walgreens gift card.
Winners must claim/accept and have signed/completed Release Forms to WLS-TV/ABC
7 Chicago no later than Friday, November 30, 2012 . Unclaimed prize will not be
awarded.
6. GENERAL RULES: No substitutions or transfer of prize will be permitted. The prize may not be sold, traded, transferred, refunded, commissioned or redeemed for cash value, except by WLS-TV/ABC 7 Chicago in its sole discretion. Winner must be 18 or older. All federal, state and local laws and regulations apply. Sponsors are not responsible for late, postage due, lost, stolen, misdirected, mutilated or illegible mail. Verification of winners is under the supervision of WLS-TV/ABC 7 Chicago, whose decision is final as to all contest related matters. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the sponsors with respect to all contest related matters. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name, city of residence and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners and guests also agree to release WLS-TV/ABC 7 Chicago, Walgreens and each of their advertising and promotion companies, and each of their respective parent, subsidiary, affiliated and successor companies from any and all claims for damage, expense, injury or harm resulting from their participation in this contest and their acceptance or use of the prize. For the name of the winner, send a self addressed, stamped envelope to ABC 7 and The Walgreens Lights Festival Sweepstakes c/o WLS-TV, 190 N. State Street, Chicago, Illinois 60601 by Friday, December 7, 2012. Void where prohibited by law.
