1. HOW TO ENTER: There are three(3) ways to enter the Sweepstakes; (1) Scan a QR Code; (2) Tap with an NFC (Near Field Communication) enabled device; or (3) Via P.O. Box.
1. Scan QR Code on Kiosk available at The Chicago 2014 Auto Show during the Promotion Period. Using your mobile device and compatible QR Code scanning application, scan the QR Code and then follow the instructions provided to enter the Sweepstakes. You must have a compatible mobile device with a suitable QR Code scanning application in order to enter. Entrants may be charged and are responsible for fees/costs of standard rates for text messages sent and received from their mobile device, other carrier-specific rates and applicable taxes, according to the terms and conditions of their wireless plan.
2. Tap an NFC (Near Field Communication) enabled device to the MTAG Kiosk available at The Chicago 2014 Auto show during the Promotion Period.
a. The Sweepstakes entrants may be given the option to receive (a) commercial e-mails from Sponsor and/or commercial e-mails from Mazda (all such information provided to Mazda is subject to, and will be treated in a manner consistent with, Mazda Privacy Statement available at MAZDA privacy), and/or (c) mobile text messages and updates from Sponsor, eligibility to participate in the Sweepstakes is not dependent upon Entrant's consent to receive such e-mails and/or mobile text messages and updates; and consenting to receive such e-mails and/or mobile text messages and updates will not impact any Entrant's chances of winning;
3. Mail in an envelope with the following information: first and last name, address including city, state and postal code, daytime phone number, and birth date. Entry to be mailed to ABC 7 and Mazda3 NEWpossible, c/o WLS-TV, P.O. Box 64389, Chicago, Illinois 60664-0389. All entries must be postmarked and received by February 21, 2014. Limit one entry per person, per trip, per mailing address (regardless of more than one person uses the same mailing address.) Sponsor is not responsible for lost, late or misdirected mail. All entries remain the property of sponsors and will not be acknowledged or returned.
No Purchase Necessary. To enter at the 2014 Chicago Auto Show: Entrants must go to the Mazda exhibit at the 2014 Chicago Auto Show at McCormick Place, 2301 S. Lake Shore Drive, Chicago, Illinois, between the dates of Saturday, February 8, 2014 and Monday, February 17, 2014. The hours are February 8-16: 10 am to 10 pm and Monday, February 17: 10 am to 8pm. Entries must be submitted via Kiosk on-site must include name, complete street address, phone number, date of birth and e-mail address.
Regardless of entry method, to be eligible, entrants will be required to provide their full name, complete street address, phone number, date of birth and e-mail address.
2. ELIGIBILITY: One (1) entry per person, per Trip Giveaway. Any individuals (including but not limited to employees, consultants, independent contractors, and interns), who have, within the past six months, performed services for WLS Television, Inc, Mazda North American Operations, any organizations responsible for sponsoring, fulfilling, administering, advertising or promoting the contest or supplying the prize, and/or their respective parent, subsidiary, affiliated and successor companies, and immediate family and household members of such individuals, are not eligible to enter or play. "Immediate family members" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses. "Household members" shall mean people who share the same residence at least three months a year. Void where prohibited. Entrants must be residents in the viewing areas of Illinois, Indiana, Michigan or Wisconsin.
3. TO CLAIM PRIZE: Three (3) winners will be selected on or about Monday, February 24, 2014. The winners will be contacted by telephone and/or U.S. mail by WLS-TV. Prize Release Forms will be sent overnight. Prize fulfillment will be coordinated between the winner and WLS-TV/ABC 7 Chicago once forms are completed and returned to WLS-TV/ABC 7 Chicago. No substitutions or transfer of prizes will be permitted. Winner must be 18 years of age or older. Unclaimed prizes will not be awarded. To claim prizes, (a) winner must provide proof of age, identity and residency; (b) winner must sign and return Affidavit of Compliance; Advertising Release and Prize Release Forms.
4. VERIFICATION: Winning entries are subject to verification. Any entry which is ineligible, mutilated, altered, duplicated, tampered with or was not obtained legitimately is null and void. All entries become property of WLS-TV/ABC 7 Chicago. WLS-TV/ABC 7 Chicago are not responsible for failed, partial or garbled computer transmissions, or for technical failures of any kind, including but not limited to electronic malfunction or damage of any network, hardware of software. If for any reason the contest is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, other causes beyond sponsor's control that corrupt or affect the administration, security, fairness, integrity or proper conduct of the sweepstakes, WLS Television, Inc. reserves the right, in its sole discretion to disqualify any individuals who tampers with the entry process, and to cancel, terminate, modify or suspend the sweepstakes. No responsibility is assumed for: any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries; or any problems or technical malfunctions of any telephone network or lines, computer online systems, servers or providers, computer equipment, software, failure or any email or electronic entry to be received on account of technical problems or traffic congestion on the Internet at any Website, or any combination thereof, including any injury or damage to entrants or any other person's computer related to or resulting from participating in or downloading any material from this sweepstakes. In the event of a dispute of an electronic entry, prize will be awarded to the name on the entry form, not the owner of the email account.
5. AVAILABLE PRIZE/OFFER: Odds of winning are based on the number of entries per Trip Giveaway received between February 8, 2014 and February 21, 2014. Unclaimed prizes will not be awarded.
• Trip 1: New York: Trip includes 3-day/2-night trip for two (2) to New York City to see "LIVE with Kelly & Michael." Winner will receive round-trip airfare from Chicago to New York, ground transportation to and from the airport/s, hotel accommodations, two (2) tickets to see "LIVE with Kelly and Michael" and a VIP Gift Bag. (Estimated Retail Value: $3,500). Good anytime through December 31, 2014 based on availability and live show schedule.
• Trip 2: Chicago: Trip includes a 3-day /2-night Staycation for two in Chicago. Winner will receive hotel accommodations, dinner for two (2), two (2) tickets to see Windy City LIVE and a meet and greet with hosts Ryan Chiaverini and Val Warner and VIP Gift Bag. (Estimated Retail Value: $3,500). Good anytime through December 31, 2014 based on availability and live show schedule.
• Trip 3: Los Angeles: Trip includes a 3-day/2-night trip for two (2) to LA
to see a "Dancing With The Stars" show taping. Winner will receive round-trip
airfare from Chicago to Los Angles, ground transportation to and from the
airport/s, hotel accommodations, two (2) tickets to see "Dancing with The Stars"
and a VIP Gift Bag. (Estimated Retail Value: $3,500). Good anytime through
December 31, 2014 based on availability.
Items NOT INCLUDED as part of trip prize package are: Hotel room service; laundry service, alcoholic beverages, merchandise and souvenirs, spa services, local and long distance telephone calls, internet service, gratuities, incidental expenses, travel insurance and all other costs and expenses not otherwise mentioned herein and are the responsibility of the winner.
Winner must claim/accept and have signed/completed Release Forms to
WLS-TV/ABC 7 Chicago no later than March 14, 2004. Unclaimed prize will not be
awarded.
6. GENERAL RULES: No substitutions or transfer of prize will be permitted. The prize may not be sold, traded, transferred, refunded, commissioned or redeemed for cash value, except by WLS-TV/ABC 7 Chicago in its sole discretion. Winner must be 18 or older. All federal, state and local laws and regulations apply. Sponsors are not responsible for late, postage due, lost, stolen, misdirected, mutilated or illegible mail. Verification of winners is under the supervision of WLS-TV/ABC 7 Chicago, whose decision is final as to all contest related matters. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the sponsors with respect to all contest related matters. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name, city of residence and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners and guests also agree to release WLS-TV/ABC 7 Chicago, Mazda North American Operations, and each of their advertising and promotion companies, and each of their respective parent, subsidiary, affiliated and successor companies from any and all claims for damage, expense, injury or harm resulting from their participation in this contest and their acceptance or use of the prize. For the name of the winner, send a self addressed, stamped envelope to ABC 7 and Mazda3 NEWpossible WLS-TV, 190 N. State Street, Chicago, Illinois 60601 by March 28, 2014.
