Maria Gonzalez has been charged with failure to report an accident causing injury, a felony, and taken into custody.
The 16-year-old was critically injured in the crash about 7:41 a.m. in the 3800-block of West 68th Street near Marquette Park.
He was then taken to Advocate Christ Medical Center in Oak Lawn.
Surveillance video showed the teen coming out of an alley before being hit in the middle of the street by a black SUV.
ABC7 Chicago froze the video before the collision.
"That could've been my kid," said Erik Diaz, who lives nearby. "I'm scared; my kid runs across the street and somebody's coming quick. That's careless, reckless and uncivilized."
"Is it something out of the ordinary? Sadly, it's not," neighbor Carlos Then said.
Diaz and others have been left devastated.
"I tried to tell the people we need speed bump here just to slow it down. But, there's always speeding right here, and it's very sad," he said.
"It might be very dangerous because cars are coming off Pulaski, just coming down real fast. There are no speed bumps here," Then said. "I don't know what would go through a person's mind. OK, it happened, but why keep going?"
CPD said Gonzalez is next due in court March 5.
