Sixty-one-year-old Samuelstine McGrew left the Columbus Park Nursing and Rehabilitation Center on South Austin Boulevard on Wednesday. She hasn't been seen since.
Police warn, if you see McGrew, she may act or appear to be confused.
Anyone with information about McGrew's whereabouts is asked to call police.
