The Chicago Fire Department is investigating one of its paramedics to see if there were improper entries made into a department computer after a 911 call.

On Memorial Day, the paramedic responded to a call on the city's South Side and may have made improper entries about treatment for someone at the home. Later in the day, paramedics were called to the same house and found a man dead.

It's not clear if the paramedic has been taken off duty.
