The crash happened on the city's Southwest Side, just before 6 p.m. last Wednesday.
Investigators say a gray or black four-door sedan with a male driver hit a 54-year-old woman who was crossing the street near Western Avenue and Marquette Road.
The driver took off, and the victim died from her injuries.
Tanja Annzionetta Safforld was identified by the Cook County Medical Examiner's office on Monday.
Police released images of the car wanted in connection with the deadly hit-and-run crash.
Chicago police continue to investigate. No arrests have been made.
Family said Monday Safforld was supposed to be making a simple run to the dollar store. But, when the grandmother didn't return home, they said they knew something was wrong.
"We were having a movie night," niece Kyara Safforld said. "And, when she didn't come back, I informed my dad, like, 'My aunt's been going for over an hour.' And, it just started from there."
Loved ones Monday returned to the intersection, just three blocks from Tanja Safforld's home, with heavy hearts.
"What our Thanksgiving go be, to whoever did this to her?" brother Thomas Safforld said. "She cooked for us every holiday. She makes our pies. We won't even have our pies this year. That's what it means to us. That's what it means."
Tanja Safforld's family didn't know where she was until a day later.
"We couldn't be there with her to say goodbye or anything," daughter Brittany Rogers said.
Now, the family is calling for justice.
"Just turn yourself in. We forgive you. Come on, this is somebody's mama, please," son Damion Martin said.
