CHICAGO (WLS) -- Metra is facing possible penalties after the state's top watchdog issued the results of a three-year investigation.

Illinois Executive Inspector General Ricardo Meza says the investigation determined Metra had a long-standing practice of falsifying work logs.

The logs are required to ensure public safety by limiting the hours of train crews. Metra disputes the report and says safety was never at issue.

Another investigation also found a Metra Police Department lieutenant violated a state ethics act.

He completed a mandatory online ethics training program for four other police officers.
