CHICAGO (WLS) -- Chicago's Weiss Memorial Hospital has lost its license, officials said Wednesday.

The Uptown hospital is currently closed, but there have been community efforts to reopen it.

Weiss has had a provisional license since last year.

The Illinois Department of Public Health says leadership has not made improvements.

The hospital can still reopen, but will now need to go through the regular hospital license process.
