Jasmine Baez, 16, was last seen about 1 p.m. on Oct. 14 near the intersection of East 91st Street and South Commercial Avenue, according to a missing person alert issued by police. She lives in the 8800 block of South Houston Avenue.
Baez was described as a black girl, standing 5-feet and weighing 100 pounds. She has brown eyes, braided black hair and a medium complexion. She requires glasses, but was not wearing them when she went missing, police said. Her right eye also "appears droopy."
She was last seen wearing a blue t-shirt, blue jeans and black gym shoes. She also wore a brown and white jacket with a square pattern on it, police said.
Baez "has the mental capacity of a third grader" and attends South Shore International College Prep School, but did not show up for class on Tuesday, police said.
