An investigation by the Indiana Occupational Safety and Health Administration determined the September 1 accident occurred because welding connecting a car to a tow cable for a stunt gave way. The weld was made by a certified welder, according to Indiana OSHA, and all the necessary safety precautions were in place.
Indiana OSHA spokesman Mark Lotter said the crash was "an unfortunate and unforseeable accident."
