Federal officials have issued a number of safety recommendations after a deadly train derailment and explosion in June 2009 near Rockford.

The National Transportation Safety Board determined the derailment took place after heavy rains caused the tracks to wash out. The board has recommended a review of storm water management systems across the country. The NTSB is also calling on the Canadian National Railway to improve safety efforts.

A Rockford woman died as she tried to escape the explosion caused by the derailment.
