The Indianapolis Colts will honor the LaPorte High School football player who collapsed during practice and later died.

The family of Jake West will take to the field during halftime of the Colts game Sunday against the Seattle Seahawks at Lucas Oil Stadium.

The 17-year-old high school junior and football linebacker died on September 25 from an enlarged heart.

His family will be presented with a Colts jersey bearing the number 26, Jake's number, after a 45 second video tribute.
